NJPACO R is a physician-owned and operated organization. NJPACO R was established to improve healthcare delivery based on Accountable Care Organizations’ principles and purpose; the “three-part aim” of improving patient experience, improving population health, and decreasing per capita healthcare costs. NJPACO R INCLUDES BOTH PRIMARY CARE AND SPECIALIST PHYSICIANS, as both are critical for coordinating collaborative healthcare for optimized outcomes.

Physicians in our ACO are working hard to give you the best healthcare experience possible through increased office hours, offered urgent next-day appointments, and additional office staff that helps improve your overall health care.

Care Coordination

Increased coordination of care typically means increased communication between ACO health care providers, improved outcomes in overall patient health care, and reduced costs to Medicare.

Patient Engagement

Patient engagement and engaging the patients’ families are key factors in improving patient health outcomes. Patient Engagement in an ACO has two primary goals: expand the doctor-patient relationship and educate the patient. ACO doctors attempt to share the decision-making process with their patients and well-informed patients are more likely to follow through with their joint treatment decision.
